Our CCM program is designed with the patient in mind. It has one simple goal: to keep the patient healthy, independent, improve care outcomes and prevent or reduce readmission/re-hospitalization. Every patient receives the care related to his or her specific condition and risk level. Pertinent information, education and responsibility is given to the patient to allow them to get involved in their own care. The program’s goal is to deliver services that improves health outcomes and reduce healthcare spending. Our program combines easy-to-use technology and nursing services. Our certified clinical teams performs the required monthly services with the goal of improving quality of life of patients.
The key features of our CCM program includes:
- Medication Therapy Management: reconciliation, review of adherence and potential interactions and oversight of patient self-management of medications. These include Personalized Medication List, Medication Review, Comprehensive Medication Review and Medication Action Plan for each patient.
- 24-hour access to care management services
- Comprehensive and coordinated care plan developed with significant input from the patient and caregivers
- Comprehensive, initial assessment including physical examination as well as wellness visits
- Monthly face-to-face quality clinical time with patients
- Promotion of self-care by utilizing our remote patient monitoring (RPM) devices from our Telehealth Department. This include weight scale, glucometers, pulse oximeters, blood pressure monitors, temperature probes, etc. for patients to capture and transmit biometric vital signs data for surveillance and monitoring by clinical staff.
- Coaching and education from trusted professionals with a goal of trying to reverse the effects of the diseases.
