- Documentation of immunity to Varicella, Rubeola, Rubella, Measles, Mumps
- Annual PPD or Chest X-Ray
- Hepatitis B Vaccines (series of 3)
- Mantoux Test*
- Banking Letter or information for Direct Deposit
- Valid email address
